- Serotonin syndrome (potentially life-threatening) — Methylene blue acts as a monoamine oxidase inhibitor (MAOI). It can cause severe CNS reactions (agitation, high fever, seizures, rapid heart rate, muscle stiffness, confusion, or death) when combined with serotonergic drugs like SSRIs (e.g., Prozac, Zoloft), SNRIs, certain antidepressants, migraine meds (e.g., sumatriptan), opioids, dextromethorphan, or linezolid.
- G6PD deficiency risk — Can trigger hemolytic anemia (red blood cell destruction) in people with this genetic condition—avoid completely if you have it.
- Contraindicated groups — Avoid if pregnant, breastfeeding (potential fetal/infant harm), have kidney issues, or severe renal failure.
Common Side Effects (Even at Low Doses)
- Blue-green discoloration of urine, stool, skin, or tongue (harmless but noticeable).
- Nausea, vomiting, diarrhea, or gastrointestinal upset.
- Headache, dizziness, confusion, sweating.
- Altered taste, feeling hot, or limb pain.
- Temporary staining of mouth/teeth.
